Every thing is fine until:
99999999 sending: 00 40 07 00 CA 00 6E 00 08 00 EB
99999999 -> 000000 6F 0B 00 00 00 00 0D 00 00 00 00 40 07 00 CA 00 6E 00 08 00 E
B
99999999 ccid_usb.c:677:ReadUSB() read failed (3/2): -7 Invalid argument
99999999 openct/proto-t1.c:214:t1_transceive() fatal: transmit/receive failed

-7 is LIBUSB_ERROR_TIMEOUT

The error is reported by libusb.
I would search for the bug in libusb or the kernel USB stack.

Rebuild libusb in debug mode and submit the log trace to the libusb project.

Bye
