I've seen this sporadically when receiving new email, reading it, then restarting mutt later or returning a few hours later and finding the email is again marked new. I haven't seen this lately though <shrugs>... maybe e2fsck cleared things up? $check_mbox_size is set here too, and seemed to have cleared some problems up... but it just might be my perception.
