Follow-up Comment #3, bug #17620 (project mypaint):
You could check whether the same problem happens in other GTK applications
like GIMP and Inkscape (after enabling pressure in these apps). You could
start mypaint with the --logfile=... option and tell us what it prints the
first and the second time you start it.
However it will probably boil down on debugging GTK on Windows to fix the
problem. I don't know of anyone who has the setup and skills and motivation to
do this, so help is appreciated.
