This is exactly my problem, 

I'm not using KDE, but GDM (ala KnoppMyth) so there is no 
/home/mythtv/.kde/Autostart/myth-load.sh to edit. I thought this was clear in 
my first post, sorry.

What's the GDM equivalent? I did a find / myth-load.sh but nothing turned up. 
Any other knop users with this issue?

As far as I know mplayer does not support dvd menus, you could try xine
which does. You might find this thread useful:
