Hi Steve,
What you are describing is a balanced patch bay that was wired in
an un-balanced config. I have seen patch bays on the older consoles that
actually only have two contacts, with the "Tip" contact being where the
"Ring" would be in a balanced jack.

ones shipped with my old Series 2, they used "balanced"
(tip-ring-sleeve) jacks and tied the ring and sleeve together to
unbalance them. When I rewired that console, I took the time (many
many hours with a desoldering gun) to remove all the shorting wires
between the rings and sleeves and wired it as a balanced patchbay.
You may be able to do that instead of trying to find more patchbay
modules.
