Have you rebuilt your driver with nv99 headers? GLDv3 is not a stable interface and has probably changed incompatible since June/July. If you have re-built, I suggest using dtrace to watch what occurs with your GLDv3 driver entry points; one of them is probably returning a failure code and thus plumbing is failing. The detach following the abortive plumb is probably simply due to a "modunload -i 0" or the background thread that runs on a DEBUG kernel.
