The washers you mentioned should have tabs on them, two bent slightly 
inward and two bent slightly outward.  The inward facing tabs are designed 
to fit into notches cast into the wheel hub.  The two outward tabs fit into 
similar notches in the nylon gear fitting inside the gear housing.  Both 
ride on the axle, so fitting them where they are supposed to go when 
reassembling everything, and getting them to stay there while to tighten 
everything down, is a fiddly piece of work.  Yours are probably not riding 
in those notches properly.
