   Addressed in cab6bb3b531d32829d9457bdeedba4bf48a61507: the identity lookup 
has its own per-entity guard. The new regression raises from entity 2’s id 
property; it failed with reads [1] before the fix and passes with [1, 3] 
afterward. Fresh verification: 194 versioning unit tests and changed-file 
pre-commit passed; independent Claude review found no publication blockers. 
This verifies Python exception isolation, not recovery from an unusable 
database transaction.

   Addressed in cab6bb3b531d32829d9457bdeedba4bf48a61507. Removed the test-only 
wrapper and moved the first-state-retention test onto the production capture 
loop. Removing the deduplication guard made that test fail; restoring it 
passed. The fresh 194-test versioning suite and changed-file checks are green.

   Addressed in cab6bb3b531d32829d9457bdeedba4bf48a61507 with the suggested 
attempted-read wording, including failures that retain nothing. Existing 
failed/mixed-read timing cases pass in the fresh 194-test versioning run. 
Metric names are unchanged; the separate namespace follow-up remains deferred.
