There are no (pseudo)SC in your document! There's condensed uppercase in the replacement font (AdobeSerifMM) in the places where they should be. You should look into the font list in the document properties in Acrobat (Reader): while there's still "Adobe Serif MM" or "Adobe Sans MM" mentioned anywhere, you've a problem.
