Minor source issues (#ifdefs) with --disable-mt and Win32
ntop.c and hash.c needed #ifdef CFG_MULTITHREADED around some HEARTBEAT lines.
webInterface.c needed the traceEvent() moved inside the #ifdef
globals-structtypes.h and icmpPlugin.c still had HAVE_GDCHART vs. the MAKE_WITH_GDCHART
option (Win32 forces #define MAKE_WITH_GDCHART)
