For anyone else, since Kurt stated that SQL was desirable... If you're looking to host a survey application yourself and the need for an external DB engine is not required/desired, SurveyGold may be the ticket. We've used it before and been pleased. The one time we did have some issues, we emailed support, and the developer was very responsive to our emails. We liked it enough that we ended up buying the upgrade a while back and used it. Since then, however, most of what we have done has been on SurveyMonkey.
