If you're looking to host a survey application yourself and the need for
an external DB engine is not required/desired, SurveyGold may be the
ticket.
We've used it before and been pleased. The one time we did have some
issues,
we emailed support, and the developer was very responsive to our emails.
We
liked it enough that we ended up buying the upgrade a while back and used
it. Since then, however, most of what we have done has been on
SurveyMonkey.

I'm not sure whether it runs on Server 03 or 08, as we ran it from an XP
workstation, but since it states that it will run on Win7, I don't know
why
it wouldn't run on Server 08.
