I haven't done it, so I'm not qualified to speak directly to the points of the plan. I will say that you'd be well advised to run through the whole thing in your virtualization environment of choice to test your plan and work out any issues. (Snapshot or backup your VMs once they're running and before you start any migration procedures.)
