Here's my clarification of the "wherever" confusion.

If I have OGC on page 12, then Somewhere in the book (page 12 or page 197) I
have to say "The text on page 12 is OGC"

That is clearly identifying it "wherever" it is.

If I had some OGC creature, and it was fully detailed on page 9, and
referred to on pages 18 and 46... Somewhere I'd have to point out that all 3
pages contain OGC.
