This paragraph seems way more verbose than it needs to be. Perhaps there 
are overriding legal reasons why you've worded it this way, but especially 
since you are spelling out below exactly what is OGC and what's PI for each 
item, I think something like this might be better:

This module uses Open Game Content from both the Creature Collection and 
Creature Collection 2: Dark Menagerie published by Sword & Sorcery Studios. 
That Open Game Content includes content designated as Product Identity, 
which is used here by specific permission. The following list details these 
item, and their designations as Open Game Content or Product Identity:

These are good. I like this.

I have to agree to Brad here. I'm not sure why you've included this 
paragraph, when the rules for reproducing OGC and PI are laid out 
explicitly in the license (which, of course, is also included).
