Refixed. It took me long time to fix because I nearly gave up trying different
possible solutions and declared the bug unfixable (until that death code
rewrite happens). It seems like people have been tinkering around with the
death code for ages, and it's never been quite right, and oftentimes worse than
it used to be in year 0.
Spawned already-dead enemies give rewards and trigger on-death spawning. If an
enemy spawns itself, it will fill up all the battle slots and then all copies
are deleted.
Also, enemies in a formation with 0hp will fade out (instead of staying around
then disappearing)
