my question, how do you check to see if a clutch fan is bad?

Just listen to it. If it sounds like a jet plane is taking off when you accelerate, it's bad.

Not necessarily. This just means the clutch is locked up. They can fail by freewheeling / failing to engage as well. The airplane taking off noise can also be caused by a clutch with the wrong rpm limiting spec. I have seen aftermarket heavy duty clutches that were incorrectly listed as being correct for a passenger car application. The car was geared a lot taller than the truck resulting in the fan churning noisily away at cruise speeds.
