After some experimentation, we were indeed able to create a new action trigger that leverages the existing penalty.PATRON_EXCEEDS_FINES hook so that it generates an email at the same time the penalty is applied to accounts. I'll work on figuring out how to put it in git for review, but in the meantime, if anyone wants a description of how we implemented it, let me know.
