You probably didn't build with --enable-transarc-paths (presuming you
are talking about binary build on openafs.org).

You need to configure with that option, as well as 'make dest'.
I did do --enable-transarc-paths, but the binary build on openafs.org puts things in /Library/OpenAFS and /var/db/openafs, not in /usr/vice, so I assume there is something else that rearranges things for the installer.

But I'm also concerned if some other option other than --enable-transarc-paths needs to be set for configure.

Can you be more specific about your fileserver problems? are you
referring to "fileserver" or the fs combination of fileserver +
volserver?
When I try to replicate the root.afs and root.cell, it fails when I try to release and fails during salvaging. I fix things up without replication, and when I try to delete a file, it takes a long time, and then the fileserver goes offline. I try to salvage, and directories are missing. I'm now at the point where /afs doesn't even respond.

Here are some errors from SalvageLog:

You can send certain signals to the file and vol servers to bump up the
debug level quite a bit. Look for CheckSignals in viced.c ( I forget
exactly which does what, and I think they are different on some
platforms. ).
OK, I'll give it a try.

I'm trying to get the AFS server functionality working under Mac OS X
(which is documented as "untested"), and have gotten fairly far with
bringing up a server and configuring it. Most things seem to work
(authentication, acls, etc), but there is something seriously wrong
with the fileserver itself. If anyone can point me at documentation or
someone who can answer my questions, I would be grateful:

1) A straightforward build of the sources does not produce the same
assembly of files as distributed in the binary release for Mac OS X.
What configuration settings and other massaging are necessary, so I am
determine if my problem is related to a misconfigured build?

2) How do I go about diagnosing the fileserver problem? A guide to the
file formats would be helpful.
