If the delay is taking place before the pioctl calls, the problem is
not going to be the shell extension.

rebooted and fired up process monitor and filtered the Path
on "_._AFS_IOCTL_._" before opening the explorer Window. There
was a bunch of netidmgr.exe process's, duration all less then
a millisecond or so. I then opened an explorer window with a
path into afs, no new process's. I right clicked, got the 40
second delay then a bunch of Explorer.EXE process's showed up,
duration all below a millisecond.
