I have succeed one time to have a working RA (with import in LDAP and MySQL), but I don't find how to reinstall i correctly ... The imports in LDAP don't works anymore. For me the configuration import stop on "Importing CA-Certificates into ldap ...". For your problem, maybe you have a bad MySQL (or the DB you use) configuration.
