This reflects our system. Records are printed, signed by clinical ppl
and counter signed by pts, if nec. For very sensitive things (meds)
only systems ppl may edit or delete records. Staff will ask HIS to
make the change. Usually an obvious error was made.
Our sister agency in Westchester County has ESOF and so HIS does not
update or delete records without forms being completed indicating why
the change was necessary (note added to wrong chart, so on...)
However, all records can be edited by *someone*.
In my view the perfect EMR will exist when we have perfect clinicians. ;-)
