I have an old dislike for Adobe Acrobat et al because it (years ago?
still does?) break with odd printer sizes, etc., but I've gotten
into the habbit of using fpdf library (free pdf) with php to generate
output destined for a printer within a web app for the very reasons
mentioned. fpdf has its own issues -- you don't have a 'BACK' button
so decisions have to be made whether to write the pdf to a popup
window, etc. and the solutions aren't quite as elegant as might
be desired -- but it works. http://www.fpdf.org/

For old fashioned apps I'm using SWT and it's printing model is still
in progress cross platform and can be memory intensive (based on
Windows paradigm of building in memory the bit image of the page
-- yuck). Combine with gcj and you break free from reliance on
a jvm. Just a data point.

I presume many others have faced similar problems - anybody know of FOSS solutions that are simple and robust without requiring a lot of fiddling by users?

We have considered using intermediate formats like PDF (save HTML to PDF and then print the PDF), but it's a hassle.
