Putty-CAC works quite well. There's also KiTTY which seems to be more
maintained than Putty is.
Nevertheless, one is open source, the other is not.

pkcs15-tool -vv crash comes from problems with debug_file.
Unfortunately debugging under windows is a general PITA.

Why pkcs15-tool -E crashes is a different story.
