I'm afraid that I can't see anything from that picture.  From the viewer 
screen, all avatars appear to be standing normally.

If I remember, your running a megaregion setup with a large number of regions, right? I don't know if that may cause terrain issues though if other avatars are standing normally that would seem less likely. But testing on a conventional region setup would be the first step.

Is there anything special about the appearance settings of this avatar? Is it unusually high or have some other extreme setting? What happens if these are changed? Does this issue always happen? What happens if they stand on a prim rather than terrain?

What do you mean by the avatar going underground?  Are they just under the
terrain or are they at co-ordinates where z < 0?
