I changed the page the sidebar template references to "index" on my
test wiki and it works fine.  It is an edit to the stock template
though, so if it got updated we would have to remember to re-add the
change.  Would keeping it called "sidebar" seem sensible to you?

The style issues are not to difficult to overcome, just requires
shortening some of the section names for the sidebar.  This wouldn't
have any affect on the pdf, since it doesn't use the formating of the
index page anyway, just the list of links.
