I don't have anything particularly useful to add other than to say I have seen 
this issue 'in the wild'. I didn't have time to investigate the issue, so I 
just set the number of threads used by the pager back to 1 to get a demo out 
the door (which obviously stops the problem occurring). I have seen the pager 
deadlock on a range of hardware, all running windows XP, and all using VPB 
generated terrain. I was only using 2 http threads so it didn't occur often 
(maybe once an hour). 

Let me know if I can help with the issue at all as I would like to try and get 
it resolved (even though I had completely forgotten about it for now).

I had it 1 or 2 minutes of moving and zooming around the globe, you have to 
move in order to keep the pager busy.
The paged .ive is about 7GB and I use 4 DatabasePager threads. 
For recognizing the deadlock I press pause from time to time in the debugger 
and look if multiple pager threads are stuck in a lock.
