I believe that there is language somewhere in the Medicare regulations that 
indicates that you can't charge differently based on payment source, but this 
brings up an important point.  This is my belief, based on a recollection that 
I can't confirm from home.  Its been my experience on the multiple lists to 
which I belong that people are quick to give and accept opinions that are 
presented without substantiation.  When its something like this, I think its 
really 
important to find out the "official" answer from source documents whenever 
possible.  The PTManager website is a great resource for that type of material, 
as is CMS' own website.  Good luck tracking down the answer.
