Chris,  in  my  opinion, the debate is not about top-down ~vs~ bottom-up
approaches.  Instead,  it's  about  occupation  as  the  PRIMARY outcome
measure versus occupation as a secondary measure.

Occupation as a PRIMARY outcome measure implies that occupation is:

                a. The reason for a referral

                b. The determinant for identifying if OT is indicated

                C. The STATED goal(s)

                c. The measure of success/failure
