Ron this thread is important but is not well identified by the subject
line. I am wondering if this thread and perhaps others where important
conclusions were reached could be gathered together in a package for
suggested reading for new members. Some of the work on the expertise of
OT might also be useful. I am really heartened by your continuing use of
the term adult phys dis. This is not to say that there are no problems
with the effective use of occupation in other areas of OT practice just
that these problems don't seem to cluster in the UE.

None of Ron's points preclude either treatment approach stated by Chris
but occupation is always the 'end' and because of our activity analysis
abilities may well be the most effective means.

Chris,  in  my  opinion, the debate is not about top-down ~vs~ bottom-up
approaches.  Instead,  it's  about  occupation  as  the  PRIMARY outcome
measure versus occupation as a secondary measure.

Occupation as a PRIMARY outcome measure implies that occupation is:

                a. The reason for a referral

                b. The determinant for identifying if OT is indicated

                C. The STATED goal(s)

                c. The measure of success/failure
