As the baby's head descends Megan, the sacrum moves out and this results in the line appearing. It is something you statr to see, in most cases, when the woman is fully dilated, so is a great visual clue as to the stage of labour she is at.

Occasionally you also see it earlier in labour if the baby is posterior and rotating past the sacrum. In this case though it does not tend ot be as long and disappears again as the baby rotates towards the mother's left hip.
