Based on what you describe, I think this is the exact issue as described in: http://packetfence.org/bugs/view.php?id=1423 However with OSX we haven't been able to reliably reproduce the issue. In fact, on a 10.6 we don't have the problem (or we had it once out of 6 tests to be exact). Could you re-do the test but take a packet capture while doing so? On OSX you can perform packet captures with Wireshark or tcpdump but you'll have to do them as root (through sudo). Let me know if you need additional instructions.
