Early USB interface chips could lock up due to ESD (electro-static
discharge). For example, the early m5xx devices had insufficient
discharge protection and would lock up. The only way to shut them down
was to disconnect the battery. This was especially bad, since the
PalmV battery wasn't removeable :(

It's not likely in your case, but something to try. Actually, like
others, my vote is some kind of hardware problem.
