I have an old pencil case from a dollar store that is in the shape of of a 
triangle; kinda like a triangle shaped tube.  Anyway I use it just like you 
say, only the case lies on the desktop so I can steady the paper with one hand 
and press on the ruler edge of the triangle case with the other.

I take a ruler with a sharp straight edge, put the card over the ruler on
the fold position required, then pull/push my finger at 45° along the edge,
followed if necessary with my finger nail.  This gives the sharpest fold,
better than a ball-point pen etc.
