One thing that is not clear, is it an Apple CD rom? Otherwise, you 
need to use CD rom extension 5.3.1 from system 7.6 (not 7.6.1 but 
7.6). It mounts my non apple Matsushita 24x scsi CD rom with a PC 
audio connection up to and inclusive  9.1 but it does not appear to 
mount an IDE CD rom, even an Apple which I tested on my Beige G3 IDE 
CD rom.

Another thing is you must be certain that "Term Power" (power to SCSI 
bus) is jumpered. This is different than Termination of scsi bus. 
Alot of Hard drives require this setting. Seagate being one of them. 
I was told this by a Seagate technician. In any case, this TERM POWER 
was jumpered when I pulled out my 12x Apple CD rom and replaced it 
with a 24x.

You could also try ChrisMac CD-ROM Ext which worked pretty good with 
OS 9.1 but found CD rom ext 5.3.1 to work pretty good.

As far as your 4x not working now, unless you have changed something 
in the Extensions like your CD rom extension or pulled any jumpers, 
it is not a good sign. Make sure all cables are secure or you didn't 
bend any pins which can easily happen if you are not careful. Make 
sure there is nothing wrong with the cable. Test each connection by 
switching CD rom and HD.
