Try the CUDA reset AFTER removing the card. Then boot without the card. 
Shut down, insert card, CUDA reset again. Don't forget to hold the CUDA 
for 10 seconds. Check the OS requirements for the card. Are you up to 
date? Scan the net. See if there are specific conflicts w/ this card and 
computer combo by visiting www.xlr8yourmac.com. I know my ATA100 card 
wont work with anything less than 8.6 and my Radeon card wont work under 
anything less than 9. Finally, test the equipment in what is called a 
'known-good' CPU, meaning a CPU with basically an OS on it and nothing 
else, or a CPU you know works well. And finally, it seems the card itself 
is your block here. The card may be bad (thus the unkown in ASP) because 
even my NON-MAC IDE CDRW shows up in System profiler, it simply reads the 
ROM name and version number...if this card was working properly or at 
least marginally ASP should have SOME info on the card, at least a ROM 
name or revision...every PC PCI card I have put into my computer has that 
info, even if it is a non mac card. Doesnt nessicarily mean that it will 
work, but at least you know the machine 'sees' the card. I am pretty sure 
all PCI cards have a ROM like this on them, I think it is part of the PCI 
specification that they do to enable 'plug-and-pray' on PC's so a PC BIOS 
will find the card.

hope these tips work out for you, if not, return the card and get a 
different one. A firewire cdrw i too cool of a peripheral to just get rid 
of:)
