In some unusual cases, it is necessary to format the drive using a 
third-party formatter, specifying "low-level format", and "write zeros", and 
then to go back and attempt to initialize the drive using Drive Setup.

I encounter this situation very rarely, and usually with drives in which the 
formatting process was interrupted before it had completed.
