Mike,
I had this identical problem of mixed up IDs.  It was just a weird.  I tried 
changing jumpers, switching HDs around, etc.  I finally used TechTool Pro 3, 
zapped the pram, rebuilt the desktop and optimized with all the boxes 
checked.  When I restarted every drive including external showed up in their 
proper positions.

You do have to make sure that all your externals use IDs other than 0, 1, 3 
or 7 and the last one is terminated.

Yes, the internal SCSI bus has a one GB HD and CD-ROM

Please understand that everything was working fine for almost three
months. I switched between the two drives as the start up drive
several times early on until I just stayed on the new Maxtor with a
new 9.1 installed up until a crash which made me decide to try and go
back to my older drive with 8.6 which had given me virtually no
problems for a long time. The Ultratek 66 card worked in every slot
but I have three PCI cards assigned in a particular order because my
newer USB card (installed about the same time) would only work
correctly in the top slot. I really have no reason to suspect the ATA
card.

Don't really know, that is part of what I was asking, too. It was my
understanding that  an ATA card, especially with only one drive
connected doesn't have termination issues like a real SCSI bus.

There are no jumpers on the card and even if there were, nothing was
changed to precipitate the problem. I did, however, switch the Maxtor
HD connector cable to the second of two available connectors on the
card but only well after the problems persisted. I suspect selecting
one connector over the other could change the drive ID.

Don't know can't do it.
The Maxtor has not been seen by the computer other than as an
unmounted (and so far unmountable) drive since it disappeared earlier
this week. As of late yesterday after attempting to mount it and a
resulting crash, my computer will not boot correctly either from my
SCSI HD or from a CD as long as the HD power connector is attached to
the back of the Maxtor. It may look like everything is loading OK but
after the extensions the screen goes to a blank desktop pattern with
only a cursor. Can't power down or do anything. If I am trying to
boot from  a system CD I get to the desktop pattern with the
date/time clock (stuck at whatever the boot time was) and the little
watch cursor...indefinitely. If I disconnect power to the Maxtor  I
can boot off my SCSI or a CD and everything works fine.

I started this reply two hours ago but stopped and did a battery of
new tests and all the basics, PRAM and even CUDA reset. Swapped to
the other (originally used) connector on the Ultratek.  No change.
