 I have installed the latest version of Apple Printer Utility, which
 supersedes LaserWriter Utility.

 My LaserWriter Pro 630 activates OK now, BUT though the test page is
 turned off, it still prints each time I start the printer.


I never turn mine off. It's got a power-saver function anyway.


 Also the 630 has begun to malfunction. It has been squeaking recently
 when feeding paper, and now it pushes the paper only partway (~7cm)
 out of the slot, where it stops and the end inside the printer is
 creased by the rollers there. If there's more than one page to print
 nothing will happen until I pull the stuck sheet out. The sheet will
 have been fixed, so it's OK apart from having a double wave bent into
 the bottom.

That sounds like a hardware problem. Do you have the manual?


I don't, but I hear there's a repair kit available for faulty rollers, which has instructions. I tried to access these rollers sopme days ago, but there are obviously hidden fasteners, and I didn't want to break anything!
