It's Microconversion Video Wizard. It requires WizardWare, Display Goodies, Super Video Startup software. I am running mine on it's own HD since it will run on 8.5, but not higher.

In a series of mystery cards. (lest ye think I run here without doing all I
know  to  do  first: I  don't [run here first, that is])
This  is  again  a PCI card. Some kind of video card. It  is a semi-sandwich
card, i.e, the secondary board is only about half the  width  of the
primary. The primary card has a Mac monitor jack and an S-video jack, the
daughter  card has  an  S-video, RCA yellow, and co-ax connectors. There is
no brand  name on any  of it  anywhere. The  daughter card has  a little tin
box that  looks t.v. tunerish to me. The main  chip on  the primary board is
an IGS CyberPro 2000   C-TS-C60422 There is also  what looks  like  a Bios
chip that has  a paper label that says VP-PCI Version 1.6.0 Microconversions
INC. The daughter  card is marked with PN X67SC01A 1997. The primary card is
marked PN X66SC03 1996 There are eight chips on the primary card. all
GLT44016-50J4. I'm guessing is  is a video capture, t.v. tuner, but the
machine I  pulled it  out of had a  dead  HD,  and  I could  not peck around
for  clues. Any light?
