To my knowledge this only happens if the computer is shut down improperly. I must also assume each time you cold boot that your drive is checked and verified. This happens before the system can actually start.

 Assess how you shut down. If you are doing it properly then other
 things must be considered.

 Don't you ever get system warnings and panels saying your machine was
 shut down improperly ?

Yes, the machine has been shut down properly every time using the menu. Nothing but the finder is running at the time of shutdown and I never get the startup warnings...

Could you have temporary files from applications which are not put away or cleaned up before shutdown.

What do the files look like ? Have you tried to open them ? Or do a file menu info check ?

Were any programs installed or settings changed before this started happening ?

How old is the system installation. Any chance of corruption ?

I can't think of any hardware issues that would cause this.
