Yes, the machine has been shut down properly every time using the menu.
Nothing but the finder is running at the time of shutdown and I never get
the startup warnings...


any other suggestions?

1) Rebuild the desktop
2) run Disk First Aid until it reports nothing wrong to be fixed (sometimes DFA takes two or three runs through the disk to fix all the problems)
