It's not got a Sonnet G3 card in by any chance has it? They are known to occasionally come up dud and cause a nightmare of issues. Touch-wood, the G3/300 in my 9600 seems fine (not that it gets a lot of loading with 2 G4s on my desk!). When I got my first Mac, a 7500, I bought a G3/375 for it and it never ran properly. I sold the lot and bought a Rev. 2 G3 B&W in the end (upgraded extensively, one of the 2 G4s I have on my desk).
