Stuck programmer's switch - try removing it.
The ROMs are damaged, poorly seated, flipped or reversed. Reseat or replace.
A clip-on enhancement to the logic board has failed, come loose, or
contacts have gone bad. Remove and reseat.
Analog board failure. Replace.
