You can insert comment lines in a PDF, which are preceeded by a "%".

I recommend placing them after or within the PDF trailer.  At the end of
every PDF file is the line "%EOF".  You may be able to simply append your
comments there.  Some PDF viewers may object to this however.  Reader 5 and
Acrobat 6 both opened a test file I just whipped up without complaint.
