An interesting area, but if we can continue the heresy a little longer...
the PDF rules require %PDF- to be the first characters, but Acrobat
accepts them somewhere near the start of the file.

The obvious interpretation is that the xref entries must be value
relative to the start of the file, but in fact Acrobat seems to
take xref entries as valid relative to the %PDF-.

This makes sense, since I suspect the main reason for this accomodation
is to allow Acrobat to silently and without complaint open PDF files
in MacBinary format, which stick a header on the PDF (and a trailer
too, potentially).
