On their website, Really Right Stuff (a maker of Arca-Swiss style QR clamps, plates, flash arms, etc.) shows a nifty solution to the monopod-tilt issue. It involves attaching one of their QR clamps to a Bogen/Manfrotto swivel, then screwing the whole deal onto the pod. It allows tilting while holding the monopod vertically.
