Thanks for the feedback. 
The slave flash (GN 30) was providing the same sort of fill as the main
light, just bouncing off the ceiling toward the tub. The on camera flash (GN
32) is not TTL, and I had pre metered the scene to arrive at f/16.
I think I will try for less general fill and use your suggestions -
modifying the reflector angles, adding foil, bounce of the wall behind the
camera.

Unfortunately my wife* has a very low patience level when it comes to my
photography. Generally I have to do everything myself, which means "seeing"
the shot beforehand, then spending some time setting it up. Only then does
my son come into it. I don't really like posed shots, so it means sometimes
I'll spend 3/4 hour setting up, only to find he doesn't do what I expected
him to do.
With this set I wanted to shoot at around f/16 because when I actually took
the shots I wasn't looking through the viewfinder and didn't know exactly
where in the tub he'd be. They were all taken with a remote so I could make
sure he was safe as well as entertain him when I took the exposures.
