This one's got dust on it, so I'd guess film - maybe your 5x4 with some incline on the front standard, as there is good DOF through the centre of the subject? Don't know enough to guess the lens, but I recall someone, perhaps you, talking about having picked up a new and very cheap lens standard a little while ago.
