Actually you don't need the 12 mm tube if you  remove the mount from
the bellows place it on the camera and then place the camera and mount
back on the bellows.  It gives you a bit more of the original image
using a 50mm lens also - enough to make it work well.

If you want to use P-TTL flash to light the original slide you need to
modify the bellows mount so it tricks the *ist D into thinking there is
an A lens set to A attached.  The way I did mine the camera thinks
there is an f1.0 A lens attached and you need to set the aperture
control on the camera to the widest aperture.  The P-TTL still works
even if you set the lens at the end of fully extended bellows to f22 (I
use this for macro shots of very small shells).

An alternative setup for copying slides that I have used is a Minolta
macro focusing rail with slide copier and small copy table setup.  It
doesn't actually have a lens mount so any brand camera can be used and
with a 50mm 1:1 macro lens you can copy from full frame down to APS
sized crop of the original.  Using an A lens you can use P-TTL flash to
light it as well.
